Private Label Surrogacy (PLS) maintains rigorous screening and qualification protocols for gestational surrogacy candidates, adhering to the highest industry standards while ensuring efficient progression through each phase. Both of our Case Managers, Natalie and Jenni, have been surrogates, and they will guide you with care and experience built from their own personal journeys.
Initial qualification phase
The process begins with an online application that takes less than five minutes to see if you qualify. If you meet our initial criteria, you’ll follow up with a strategic consultation with our intake coordinator to establish clear expectations and review our evidence-based methodology for screening.
Medical documentation review
The protocol includes thorough analysis of obstetrical records, coordinated through the administrative team. Our on-staff physician then conducts an in-depth review of your records, providing detailed recommendations to the reproductive endocrinologist. Formal medical clearance from the candidate’s healthcare provider is required to proceed.

Psychological and legal screening
The psychological evaluation involves both the candidate and their partner, if applicable, in a professional assessment setting. You’ll be introduced to a licensed mental-health professional experienced in screening surrogates, with the option to do this from the comfort of your home via Zoom. We recommend doing the call during nap time or when you have a babysitter if you have children at home, to ensure your full attention. Just be yourself — no need to be nervous. Expect to be asked about the reason behind your desire to be a surrogate, what your childhood was like, and what your current support system looks like. Standard protocol includes a criminal background verification for all adult household members.
Matching protocol
Following successful completion of all screening elements, we begin the matching process with intended-parent partnerships. The case-management team handles all introductions with detail and care, and coordinates subsequent interactions. We’re here to support you from your initial intake review through well past delivery.

Medical implementation
Once matched with your intended parents, we begin working with a fertility center of their choice. First and last appointments are at the IPs’ fertility center; the agency helps source local providers for the “in-between” appointments for labs and ultrasounds. The protocol typically involves four detailed appointments that can take place closer to you for your convenience. If you have a partner or live-in spouse, they will be part of the screening process.
Your first appointment will be for labs and an ultrasound. Your medications will be sent to you via FedEx with instructions from the fertility center. The medication regimen can include:
- Subcutaneous injectable medications
- Oral pharmaceutical protocols
- Transdermal applications
- Vaginal suppository medications
- Intramuscular preparations for embryo transfer
Treatment protocols are established by the reproductive endocrinologist, with the case-management team providing thorough coordination and support throughout.
